Written Notice of Revocation, Suspension or Denial Required

PURPOSE: This rule requires the director to notify applicants or licensees in writing of the action s/he intends to take or has taken and the reasons for the action. The director, on any revocation, suspension or denial of any license, shall inform in writing, the person applying for or holding license(s) of the action the director intends to take or has taken and the reason for the action.
