PURPOSE: This rule prescribes the forms adopted and approved for filing with the commissioner. (1) The following forms have been adopted and approved for filing with the division: (A) C-4 Broker-Dealer, Sales Representative Statutory Bond (Broker-Dealer Fidelity Bond (Use commercial bond form)); and (B) C-16 Application for Renewal Registration As Sales Representative.
