Professional Fund-Raiser Organization and Employment Statement—Solicitor PURPOSE: This form is adopted and approved for filing as the registration statement for professional fund-raiser organizations. Each professional fund-raiser organization and solicitor required by sections 407.450 through 407.478, RSMo, to file a registration statement or employment statement shall file a registration statement or employment statement on the form designated in 15 CSR 603.020(2). The most current version of the registration statement form or employment statement form for use by professional fundraiser organizations or solicitors may be obtained by mailing a written request to the Missouri Attorney General, Charitable Organization Registry, PO Box 899, Jefferson City, MO 65102 or in person at the Missouri Attorney General’s Office, Supreme Court Building, Jefferson City, Missouri, or, if technically feasible, by downloading a copy of either form from the Attorney General’s Internet website.
