Forms to be Used

PURPOSE: This rule provides for the orderly transmittal of data and information necessary for the administration of the retirement system. (1) Applications for retirement allowances, disability benefits, withdrawals of funds, death benefits, service credit purchases, reinstatements and other matters concerning the adminis tration of the retirement system shall be submitted on forms provided by the retirement system, and shall be filed with the executive director or the executive director’s designee.
