Name and Address Change
PURPOSE: This rule requires licensees to
inform the Office of Statewide Electrical Contractors of any name or address changes.
(1) Licensees shall keep the division
informed of his or her legal name, mailing
address, and telephone number. Any change
shall be reported to the division in writing
within thirty (30) days.
(2) A change in legal name shall include a
copy of the appropriate documentation verifying the name change.
