Application for Health Service Provider Certification

PURPOSE: This rule outlines and standardizes the procedures followed by the committee in receiving and considering information relative to an applicant’s qualifications for health service provider certification. (1) Applications for a health service provider must be made on forms provided by the State Committee of Psychologists and accompanied by the appropriate fee.
