294.054. Contents and form of work certificates. — 1. The work certificate shall show (1) The age, sex, place and date of birth and place of residence of the child; (2) The name and place of residence of the child's parent, guardian or custodian; (3) The name and address of the employer; and (4) The nature of the employment for which the work certificate is issued. 2. Work certificates authorizing the employment of a child during nonschool hours shall be of a form and color distinct from those authorizing full time employment. (L. 1957 p. 525 § 8)
