Address and Phone of Parties. Every party to a controverted or non-controverted case must keep the Commission informed of that party’s current address, e- mail address and telephone number. Attorneys representing a party in any claim shall also keep the Commission informed of their current address, telephone number, facsimile number, and e-mail address. All self-insured employers, workers’ compensation insurance carriers, and third party administrators shall also maintain a single address, e-mail address and telephone number on file with the Commission for the purpose of electronic service, notice, and/or other correspondence with the Commission. Claimants, either self-represented or through their attorney, must provide the Commission with their current mailing address, e-mail address and telephone number. All of the above have a continuing obligation during the claim to notify the Commission of any changes in their required contact information. The most recent contact information on file with the Commission shall be presumed correct unless the Commission is notified otherwise in writing. This Rule shall be in force and effect on and after January 18, 2018.
