Eligibility

Eligibility A. Pharmacy disease management services are not covered for beneficiaries in long term care facilities or for beneficiaries receiving home health services. B. Neither OBRA-mandated counseling nor JCAHO-mandated institutional discharge counseling qualify as a pharmacy disease management service. C. Pharmacy disease management services are available to the parent or other responsible guardian when the beneficiary is a minor and/or mentally challenged and living at home.
