Mental Health Targeted Case Management Activities A. Targeted Case Management Services are defined as services that provide information/referral and resource coordination for a person and/or their family, or other supports. Targeted Case Management Services are directed towards helping the person maintain the highest possible level of independence. Case managers monitor the individual service plan and ensure team members complete tasks that are assigned to them, that follow-up and follow-through occur, and help identify when the person’s team may need to review the service plan for updates if the established plan is not working. B. Targeted Case Management may be provided face-to-face, virtually, or via telephone. Targeted Case Management is not designed to be a mobile service, but there is no prohibition on services being provided in a location other than the certified provider’s office-based location.
