Program of Assertive Community Treatment Required Services A. Operating as a continuous treatment service, the PACT Team must have the capability to provide comprehensive treatment, rehabilitation, and support services as a self-contained service unit. Services must minimally include the following: 1. Service Coordination/Individual Treatment Team 2. Crisis Assessment and Intervention 3. Symptom Assessment and Management 4. Medication Prescription, Administration, Monitoring, and Documentation 5. Co-Occurring Substance Use Services 6. Work-Related Services 7. Activities of Daily Living (ADLs) 8. Social/Interpersonal Relationship and Leisure-Time Skill Training 9. Peer Support Services 10. Community Support Services 11. Education, Support, and Consultation for People’s Families and other Major Supports
