Appeals Related to Certification A. Any agency provider holding certification by DMH may appeal the following decisions and/or penalties: 1. CRC decisions; 2. Any financial penalties invoked by DMH associated with noncompliance with the DMH Operational Standards and/or audit findings; 3. Termination or Revocation of Certification and revocation decisions; and/or 4. Denial of a complete application to maintain DMH provider certification (i.e., renewal application). B. Any agency provider applying for certification by DMH may appeal the following decisions and/or penalties: 1. Denial of a complete application to become a DMH-certified agency provider. C. Appeals not received in the timelines as outlined below will not be processed.
