A. All Applications and applicable fees; initial; upgrade; renewal and examination/training are
completed and submitted via the DMH prescribed online system.
B. Print copies of applications and other information/material are no longer available.
C. Submission of applications and/or other material via Postal Service or in person is not
allowable.
D. This document and the online system are made available online at the agency (DMH) website.
