REPORT OF WELL STATUS. Each producer or operator of an oil or gas well shall furnish for each month a "Monthly Individual Well Status Report," setting forth complete information and data indicated thereon. Such report for each month shall be prepared and filed, according to the instructions on Form No. OGB 9-12-15-Z, on or before the first day of the second month following the month during which the production was made.
