PRODUCER'S MONTHLY REPORT

PRODUCER'S MONTHLY REPORT. Each operator or producer of oil or gas
shall furnish for each month a "Producer's Monthly Report," setting forth complete information
and data indicated thereon. Such report for each month shall be prepared and filed with the
Board on Form No. OGB 9-12-15-Z, according to the instructions on said form, on or before the
first day of the second month following the month during which the production was made.
