FLUID INJECTION REPORTS. Every person injecting gas or fluid into the earth shall make a monthly report to the Board on Form No. 14, provided by the Board, showing the quantities of all oil, gas and water injected during the month covered by the report, the injection pressure and injection rate for each injection well, and identifying the underground reservoir or reservoirs into which the injection is made. In addition, those injection wells operating pursuant to Subsection 63(7)(B) shall also report the tubing-casing annulus pressure. Such reports for each month shall be prepared and filed according to instructions on the form on or before the first day of the second month following the month for which the report is being filed.
