Acceptable Identification Documents List Any applicant applying for a lost or stolen driver license or identification card is required to submit one primary document and one secondary document from the following lists. A primary document must contain the full name and date of birth and must be verifiable. If applicant does not have any of the listed documentation, you must check to see if we have a photo on file for that person. If so and you can verify that they are the same person, then you may issue the replacement.
