Contract Modifications Other Than Changes in Payment Schedule. All requests for modification(s) in the MPACT Contract must be made, in writing, to the State Treasurer and must be submitted along with any applicable Administrative Fees and any supporting written documentation required by the State Treasurer. Requests for modifications, may include, but are not limited to, a change in Contract Purchaser or transfer of the Contract benefits to an eligible Substitute Beneficiary.
