Rollovers from QTP’s (529 Plans) Operated by Other States to MPACT. It is the responsibility of the Purchaser to notify the other QTP and request that the proper amount be sent to MPACT. Purchasers should also complete, sign and transmit to MPACT the official MPACT Rollover Form as notification to expect the rollover payment.
