Reinstatement: Manufacturer

Reinstatement: Manufacturer.
A person whose Manufacturer certification has been revoked, pursuant to statutes or regulations,
may apply to the Division for reinstatement as a Manufacturer no sooner than 2 years after the
effective date of the revocation. Reinstatement of a Manufacturer certification shall include:
1.
An application, fee and statement (if applicable) that no activities took place after
certification was revoked.
2.
Provide documentation that the Applicant has satisfactorily completed any remedial
actions required as a result of the revocation. Remedial actions including, but not limited
to, notification by Third-Party Certifier that manufacturer has corrected all deficiencies,
updating or modifying training procedures, and correction to components of any
registered product as may be specified in the suspension notice.
