Hospitals and nursing homes: On the first day of every month, each institution shall make a report of all deaths occurring in or enroute to the institution during the preceding month, to the Office of Vital Records on forms prescribed and furnished by the Department of Health. If there were no deaths during the month, a report shall be filed to this effect.
