Use of Department Forms and Instructions

Use of Department Forms and Instructions. The Department may prescribe forms, formats, filing instructions, checklists, and other administrative materials necessary to implement this Manual. Applicants and other persons filing materials with the Department shall use Department- prescribed forms and comply with Department filing instructions. Department forms and instructions shall not supersede applicable statutes, this Manual, or the State Health Plan.
