Determination of Reviewability Processing Fee

Determination of Reviewability Processing Fee.
A fee of Two Thousand Five Hundred Dollars ($2,500.00) shall accompany each
application for a Determination of Reviewability and is payable to the
Department by check, draft, or money order. No application for a Determination
of Reviewability shall be processed until the required fee is received.
