Hearing Request — Requirements. A request for a hearing during the course of review shall be submitted in writing and shall: Page 33 — Mississippi State Department of Health 1. Identify the party requesting the hearing; 2. Identify the application or action being challenged; 3. State the basis for the request; 4. Include sufficient information to identify the issues to be considered; and 5. Be accompanied by the required hearing fee deposit specified in Rule 5.3. A request not accompanied by the required fee deposit shall not be deemed timely filed.
