Responsibilities of Residents of DAA Treatment Centers Upon Departure. Upon departure, the individual must complete and return the change reporting form provided by the DAA Treatment Center and return it to MDHS within 10 days to notify the agency of the individual’s new address. The change reporting form is also available and can be uploaded on the MDHS website.
