Written Request for a Subgrant Administrative Hearing Review. A Subgrantee’s written request for a Subgrant Administrative Hearing Review shall include the following: A. The name of the Subgrantee; B. The adverse action the Subgrantee seeks to dispute; and C. A signature from a representative of the Subgrantee. Written requests for a Subgrant Administrative Hearing Review shall be sent via email to subgrants.hearings@mdhs.ms.gov.
