Supervision Reports – The supervising worker must provide a Supervision Report

Supervision Reports – The supervising worker must provide a Supervision Report at least once every ninety (90) calendar days following receipt of ICPC-100B by the state ICPC office unless a different timeframe is set for Supervision Reports on ICPC-100A. The Supervision Report must contain the following: 1. The date and location of all visits with the child since the last Report; 2. A summary of the child’s circumstances, including a statement on the child’s safety and well-being; 3. A summary of the child’s academic performance, if applicable, including a copy of the child’s report card, IEP, or other educational evaluation; 4. A summary of the child’s health and medical status, including the dates of any health-related appointments, the names of any providers, and copies of any health-related evaluations or reports; 5. An assessment of the placement and caretakers; 6. A description of any unmet needs; and 7. The supervising worker’s recommendation on continuation of the placement, if applicable.
