Written Notification Requirements – The Partner Provider must provide written notification to MDCPS within ten (10) calendar days of occurrence of any of the following: 1. A change in the executive director or program director. 2. Suspending or closing of any youth living facility. 3. Major remodeling, structure or maintenance changes. 4. Change in fees or charges for services.
