Youth Placement Requirements – MDCPS will select an appropriate facility for a child and document in the case record the following: 1. The child’s level of development, social and emotional needs and the reason the child needs a group living experience; 2. The child’s Individual Service Plan; 3. The parent-child relationship and the potential for parental, foster parent, or guardian participation in the program and visitation; 4. The plan for sibling visitation if not placed together; 5. Documentation on why siblings are not placed together and plan to reunite siblings; 6. The reason the residential Partner Provider was selected as the most appropriate for the child; 7. A statement regarding the placement’s close proximity to the child’s family and county of jurisdiction.
