ARM 24.156.1304. INITIAL LICENSE

(1) Each application for an initial license as a nutritionist under the act must be accompanied by: (a) a completed application form; (b) the initial license fee; (c) a copy of the registration by the commission; (d) a copy of the diploma or a complete transcript from an accredited college or university in the field of dietetics, food and nutrition or public health; (e) a recent photograph, taken within one year of application.
