ARM 24.156.1404. APPLICATION FOR LICENSURE

(1) All applications shall be made on a printed form provided by the board and no application made otherwise will be accepted. Each applicant must provide the names of three references who are knowledgeable as to the applicant's moral character and competence as an acupuncturist. Each application shall be accompanied by a recent photograph of the applicant which has been signed by the applicant and dated as to when taken. Each applicant shall submit a sworn affidavit that he is reasonably able to communicate verbally and in writing in the English language.
