(1) A case manager shall conduct a face-to-face visit with the parent or legal representative, at least monthly, for the purpose of reviewing any need for change in the plan of care, based on the changing needs of the child or the family. (2) A case manager must have a four year degree in a human services field, three years of experience in working with children with developmental disabilities, and current family support specialist certification.
