(1) Chiropractic services are limited to evaluation and management office visits, manual manipulation of the spine, and x-rays to support the diagnosis of subluxation of the spine. (2) There must be documented medical necessity for chiropractic services by the child's primary care provider and the treatment must be directly related to a neuromuscular diagnosis of the spine.
