30-13-207 . Application for renewal of assumed business name. An application for renewal of registration of an assumed business name must be executed and delivered to the secretary of state. The application must include but is not limited to the following information: (1) the complete assumed business name; (2) the name and business mailing address of the applicant; and (3) a description of business transacted.
