Application for and nontransferability of school-age child care program license

1. An applicant shall submit an application for a license to the department. Application must be made in the form and manner prescribed by the department. 2. A license issued under this chapter is nontransferable and is valid only for the premises indicated on the license. 3. An application for a new license must be filed upon change of owner or location. 4. The department may not issue more than one in-home registration, self-declaration, or license per residence. A residence means real property that is typically used as a single family dwelling.
