Closure for reasons other than ineligibility

The division shall close a case when an individual is unavailable during an extended period of time for an assessment to determine eligibility and vocational rehabilitation needs or when an individual is unavailable for an extended period to participate in planned vocational rehabilitation services. The division shall make good-faith efforts to contact the individual and to encourage the individual's participation. Closure under these circumstances requires: 1. Documentation of the rationale for closure in the record of service; 2. Written notification of the closure decision; 3. Written notification of mediation, appeal rights, and informal review, including the name and address of the division's chief of field services with whom an appeal may be filed, and written notification of the availability of and how to contact the client assistance program; and 4. An individualized plan for employment amendment if appropriate.
