Care of adolescents in opioid treatment program

For a patient younger than eighteen years of age to be eligible for maintenance treatment in the
opioid treatment program, the patient is required to have had two documented attempts at short-term
detoxification treatment or drug-free treatment within a 12-month period. The program physician or
other health care professional shall document in the patient's record that the patient continues to be or
is again physiologically dependent on narcotic drugs. A patient under eighteen years of age may not be
admitted to a maintenance treatment program unless a parent, legal guardian, or other person
authorized by law to consent to treatment for the patient completes and signs consent form, "Form FDA
2635 Consent to Methadone Treatment".
