Application for approval of a training or retrainingprogram

An otherwise eligible unemployed individual who has filed or files a claim for benefits may make written application to the bureau for approval of a training or retraining program which the individual proposes to attend or to which the individual has been referred by the bureau. The application shall be made on forms prescribed by the bureau and shall include the following information: 1. The individual's most recent employer and employment. 2. The proposed training or retraining course. 3. The educational institution at which the individual would receive training and retraining.
