1.
An agency shall submit to the department a plan of correction addressing the areas of
noncompliance with the licensure requirements of this chapter.
2.
A plan of correction must include:
a.
How the corrective action will be accomplished for those patients found to have been
affected by the deficient practice;
b.
How the agency will identify other patients or services in the agency having the potential
to be affected by the same deficient practice;
c.
What measures will be put into place or systemic changes made to ensure that the
deficient practice will not recur; and
d.
How the agency will monitor its corrective actions to ensure that the deficient practice is
being corrected and will not recur.
3.
A plan of correction is required within ten calendar days of the receipt of the deficiency
statement and is subject to acceptance, acceptance with revisions, or rejection by the
department. Failure to submit an acceptable plan of correction may result in a directed plan of
correction.
4.
Corrections must be completed within sixty days of the survey completion date, unless an
alternative schedule of correction has been specified by the department.
