Recertification

To be recertified after voluntary withdrawal or discontinuation of certification, a laboratory shall
submit an application meeting the requirements for:
1.
A revised application, if reapplying within one year of the date that certification was
discontinued; or
2.
An initial application, if certification has been discontinued for more than one year.
