Application for hearing

In any proceeding instituted upon application, the application must be signed by the applicant or by the applicant's attorney. An application shall state: 1. The name and general description of the common source or sources of supply affected by the order, rule, or regulation sought, if any, unless the entire state is affected, in which event the application shall so state, and the statement shall constitute sufficient description; and 2. Briefly the general nature of the order, rule, or regulation sought in the proceedings.
