Feldvebel Deputy Commissioner Notice Regarding CHAPTER INS 3800 – MEDICAL PROFESSIONAL LIABILITY INSURANCE MANDATORY REPORTING OF DETAILED CLAIM INFORMATION April 3, 2013 Claim Report Form Electronic Version Ins 3800 specifies that quarterly reports are due at the Department on or before January 10, April 10, July 10, and October 10. New Hampshire is pleased to provide an electronic Claim Report Form for these required quarterly submissions. Please note that the reporting requirements are not changing. The fields on the attached file are exactly the same as those shown on the Medical Professional Liability Insurance Claim Report which has been available on our website since 2006. Carriers should submit the same information, in the same timely manner. We are simply providing an alternative method of submission which insurers can choose to use if they wish to submit data electronically. Data can be submitted electronically by emailing the report, or a hardcopy version can be submitted as insurers have done in the past.
