A. In general. A children’s court action is commenced by filing a petition with the court. Upon the filing of the petition, the clerk shall endorse thereon the time, day, month and year that it is filed. B. Abuse and neglect proceedings; party information sheet. A completed party information sheet substantially in the form approved by the Supreme Court shall accompany a petition or amended petition filed with the court that alleges the abuse or neglect of a child.
