Section 16. Service Of Notice

A. Notice shall be deemed to be properly served when the original or a true copy of the inspection report form or other written notice has been delivered personally to the permit holder, the permit holder’s agent, or a qualified operator; or when such notice has been sent by registered or certified mail to the last known address of the permit holder or qualified operator on file with the department. B. A copy of such notice shall be filed in the department’s records.
