Rule 42 . Affidavits.
1. All affidavits shall contain sufficient factual information within the personal knowledge of the affiant; such facts shall be specific and shall adequately support the relief requested. (a) A party shall provide a fact-specific affidavit to the court when extraordinary relief is sought; including, but not limited to the dates of incidents, descriptive facts and specific harm caused. (b) Extraordinary relief includes, but is not limited to an application to take a person alleged to be a person in a mental health crisis into custody, a motion for service of process by an alternative method, or a motion for order to show cause. 2. The fact-specific affidavit required by Rule 42(1)(b) shall be filed concurrently with a motion for order to show cause and shall, at minimum, state the title and filing date of the order the moving party claims has been violated, the date and method of service of the order on the party alleged to be in contempt, and specific facts that support each claim, as set forth in the motion, the party allegedly in contempt violated the order.
