Evidence of Insurance - Fleet Insureds

GATES Commissioner of Insurance [LOGO] # DEPARTMENT OF COMMERCE INSURANCE DIVISION 201 South Fall Street Carson City, Nevada 89710 (702) 885-4270 BULLETIN 88-003 SEPTEMBER 23, 1988 # EVIDENCE OF INSURANCE - FLEET INSUREDS Effective January 1, 1989 the Registration Division of the Department of Motor Vehicles and Public Safety and various Nevada law enforcement agencies will require the "Card Constituting Evidence of Insurance" as described in NAC 690B.010 - 690B.050, inclusive for vehicle registration and subsequent proof of insurance. It is requested that each insurer or insurer group furnish a copy of their currently approved "Card Constituting Evidence of Insurance" to Ms. Cindy Clampitt, Supervising Registration /Title Technician, Registration Division located at 555 Wright Way, Carson City, Nevada 89711-0740. Nevada insurers and agents are hereby advised to complete the card for Fleet Insureds so that the vehicle identification number and name of insured on the card can be correlated with the registered owner. Specifically, the vehicle identification number on the card should either show the vehicle identification number or the words "Fleet Insured". The name of insured entry on the card should include at least one name in common with names on the registration certificate so as to correspond with the license plate number.
