Application for distributor’s license: Required information.

1. An applicant for a distributor’s license must complete and file with the Director all forms required by the Director. The applicant must answer all applicable questions on the forms and must furnish the Director with any other information that the Director requires. 2. The application must include a designation of each marketing area in which the applicant intends to distribute dairy products.
