Submission of documents to Board: Signature required.

1. Any document submitted to the Board by a licensee or an applicant for a license to practice medicine, to practice as a physician assistant, to practice as an anesthesiologist assistant, to practice as a practitioner of respiratory care or to practice as a perfusionist must bear the original signature or an authenticated electronic signature of the licensee or applicant. 2. The Board may refuse to accept any document submitted by a licensee or an applicant for a license that does not comply with the requirements of subsection 1. 3. As used in this section, โdocumentโ means any written submission, notification or communication, including, without limitation: (a) An application for a license; (b) A request for renewal of a license; (c) A request for a change of status; or (d) A notification of a change of address.
