Biennial submission of self-inspection form to Board by licensee.

Each licensee shall biennially submit a self-inspection form prescribed by the Board. The self-inspection form must be received by the Board on or before December 31 of each even-numbered year. The Board will reject a self-inspection form submitted pursuant to this section if the form is incomplete.
