Proof of name change.

An applicant who has a name which is different than that which appears on any form or other documentation submitted to the Board with his or her application must provide to the Board proof of a name change, including: 1. A copy of a marriage license, showing both last names; 2. A copy of divorce papers, showing that a last name has been restored; 3. A copy of citizenship papers, including the front and back of those papers; 4. An original computerized printout from the Department of Motor Vehicles, or a similar agency from another state, listing all of the names, including aliases, of the applicant; or 5. A signed court order showing a name change.
